Billing and Insurance Information

Dr. Hoyt’s office participates with most Maryland insurance plans. Unfortunately we do not accept Medicaid or any Medicaid HMO plans.  As at most physician’s offices, patients are expected to be familiar with the requirements of their plan.  They are expected to have the appropriate referral information and copays at the time of their visit. Your appointment may be cancelled if your insurance requires a referral that was not obtained prior to your visit. In addition, the referral generated by your primary care most likely will need an actual referral number listed in order for insurance to process your claim. Most insurances will not accept a referral done afterwards, this is why the information is required beforehand.

Please be advised that if you are having a hearing test this will generate two claims; one for the office visit and another for the hearing test.  We will bill your insurance for both of the provided services.  If your insurance plan also requires a copay for the hearing test, a bill will be sent to you for the balance due.

Any surgical procedures performed will generate billing from multiple providers. In addition to a claim filed by our office for Dr. Hoyt’s services, you will also receive separate billing from the facility where the procedure is being performed, as well as the anesthesia provider’s services, and any pathology services.
