While hearing loss can affect people of any age, hearing dysfunction has been shown to affect 25% of those between the ages of 65 and 74; while 50% of those over 75 years of age have some degree of hearing loss. A variety of disorders can affect hearing. Some involve structural changes in the ear itself such as perforations of the ear drum, growths (such as cholesteatomas), or scar tissue formation as a sequelae of repeated infections. Systemic diseases such as hypothyroidism, Paget’s disease and congenital syndromes can also affect hearing. Finally, various medications and environmental factors such as exposure to loud noise can damage hearing as well. Tinnitus is the perception of sound when there is no external source and it affects 10-15% of adults in the United States. It can be described variously as ringing, chirping, clicking, or pulsations. Treatment options include hearing aids, sound therapy, and cognitive behavioral therapy. Antidepressants, anticonvulsant and anxiolytic medications have been used with various degrees of success. Dietary supplements such as ginko biloba have both antioxidant and antihypoxic properties. Ginko, as well as zinc, and melatonin have shown variable effects on tinnitus. Some studies have shown a benefit, while others have shown little change in symptoms with these medications. They include inflammatory conditions such as infections, congenital or developmental anomalies, as well as benign and malignant neoplasms. There are a variety of ways that an accurate diagnosis can be made. Physical examination, CT scans, ultrasound, and both incisional and excisional biopsies may be necessary. A number of these options can be performed conveniently during the office visit. Treatment options vary widely from observation to removal of the growth. Malignancies are sometimes treated with a variety of therapies including surgery, radiation therapy, and chemotherapy. Some treatments can be performed locally. The act of swallowing is a very complicated process in which a number of muscle groups must contract in a very specific sequence in order to propel a bolus of food or liquid appropriately through the throat and into the esophagus. Neuromuscular disorders, strictures, acid reflux disease, and tumors can all interfere with this process. There are a variety of ways to assess swallowing function. Endoscopic evaluation in the office as well as with a speech therapist can be especially helpful. Radiologic swallowing studies can be helpful as well. Sometimes other physicians such as gastroenterologists may be consulted for more complicated problems. Once a diagnosis is made, a variety of treatment options are available. Medical treatment of underlying neuromuscular diseases such as Parkinsonism may prove effective. Aggressive reflux measures can be especially helpful as well. Surgical options including dilation of strictures, and removal of obstructing growths, are sometimes also necessary. Feelings of lightheadedness, instability, a spinning sensation, or a sensation of abnormal movement can indicate a balance disorder. Balance is controlled by various body systems. These include the inner ears, the eyes, the nerves and muscles in your extremities, and your brain. Information from these various systems is integrated and interpreted by your central nervous system. Your brain then sends messages to your extremities and muscles in an effort to keep your body appropriately oriented in space. Problems with any of these organ systems can result in balance difficulties. In addition, other organ systems such as your cardiovascular system can have abnormalities (such as arrhythmias and poor circulation) which can cause similar feelings. Certain diseases such as anemia and hypoglycemia can also be causes of disequilibrium as well. Sometimes a diagnosis can be made with an office visit and a history and physical examination. More specialized testing may sometimes be necessary. It may include blood work, a magnetic resonance imaging study of your brain, and certain audiologic tests. Our practice employs both electronystagmography (ENG) and well as videonystagmography (VNG) equipment to better assess central nervous system functioning, as well as inner ear functioning, in patients with balance disorders. It is performed at both of our practice locations. Treatment for patients with balance issues may include physical therapy, medication, and rarely surgery. About 30% of people will experience an episode of hoarseness at some point in their lives. Higher rates affect certain professions more commonly, such as teachers. A variety of ailments can affect the larynx (voice box) and cause hoarseness. Voice abuse such as loud or prolonged talking, gastroesophageal reflux, smoking, post-nasal drainage from allergy or sinus issues, and generalized neurologic disorders can all affect the larynx and cause abnormal voice issues. Structural problems with the larynx such as vocal cord nodules, polyps, tumors, and cysts can also be issues as well. The most efficient way to evaluate voice issues is to examine the voice box itself. This can be done with either mirrors, or a small flexible tube which can easily be inserted through the nose to examine the lower throat. Depending on the results, further voice analysis, or more sophisticated visualization may be necessary. While many causes of hoarseness are self-limited, there are a variety of treatment options available. Changes in medication may be effective. Treatment of reflux disease can be helpful as well. Surgery to remove abnormal growths, as well as to reposition vocal cords, can significantly improve voice quality. Treatment of tumors can frequently result in improved voice quality as well. They aid in the immune response to certain organisms such as bacteria and viruses. Frequent infections, however, can overwhelm their processing ability and make them ineffective in their activity. As such, they can cause illness in certain individuals. Tonsillectomy (and adenoidectomy) are performed for a number of different reasons, but sleep disordered breathing and recurrent infections are the two most common reasons. While the frequency of tonsillectomy in the treatment of throat infections has declined over the years, more tonsillectomies are performed today because the frequency of tonsillectomy for treatment of sleep disordered breathing in children has increased substantially. Today a tonsillectomy is performed about 500,000 times a year in children under the age of 15 in the United States. Sleep disordered breathing represents a range of disorders varying from snoring to sleep apnea. About 10% of children snore, while 1-4% of children have sleep apnea. Approximately 40% of children with some degree of sleep disordered breathing will exhibit behavioral problems including agitation, inability to concentrate, enuresis, and depression. Obstructive sleeping patterns can also interfere with appropriate hormone responses and can be a reason for growth retardation. Rates of throat infections for which a tonsillectomy would be appropriate are approximately 7 infections in a year, or five infections a year for two years, or three infections a year for more than two years. It can be caused by both viruses and bacteria. Viruses causing colds are much more frequent in children than in adults. While most adults can experience between 2-3 colds a year, children can average between 6-21 upper respiratory infections a year. Since between 0. 5% and 5. 0% of upper respiratory infections are complicated by sinus infection, children can be more prone to sinus infections. Common symptoms of children with sinusitis include a low grade fever, bad breath, a prolonged cough, and nasal discharge. A variety of tools are available to aid in the diagnosis. Nasal endoscopes and CT scans are both used to help diagnose sinus conditions in children. Treatment options include both medicine and surgery. Prolonged courses of antibiotics with decongestants, saline nasal rinses and antihistamines (when appropriate) help to clear the majority of pediatric sinus infections. Rarely children may need surgical procedures such as an adenoidectomy, sinus irrigation, or sinus surgery, to clear more persistent sinus infections. Allergic Rhinitis Allergic rhinitis is the most common chronic medical condition of children in the United States. The incidence increases throughout childhood from less than 1% of infants, to 5% of 5-9 year olds, and 9% of 16 year olds. Approximately 2% of colds will progress in this manner to become bacterial sinus infections. While most of these can be treated effectively with antibiotics, some patients may suffer from prolonged, or repeated infections. In those cases more aggressive treatments may be necessary. Effective treatment of allergic disorders, avoidance of environmental irritants, and correction of nasal anatomic deformities are all effective options in reducing the severity of sinus symptoms. While few are enthused about surgical treatments, there are currently a variety of options now available that reduce the amount of tissue removal and therefore they are more tolerable by patients, and shorten their recovery. Allergic rhinitis is the fifth most common chronic disease in the United States. Its symptoms include nasal congestion, nasal discharge, itch or watery eyes, and sneezing. The symptoms may have a seasonal variation, or they may be present on a year round basis. The symptoms are generally caused by the body’s response to inhaled substances. Various tree or grass pollens, as well as cat or dog dander, dust, and mold are common stimulating factors. Musicians Earplugs and Swimming Molds Conventional ear plugs can provide up to 40 dB in sound reduction. They are expected to have the appropriate referral information and copays at the time of their visit. Your appointment may be cancelled if your insurance requires a referral that was not obtained prior to your visit. In addition, the referral generated by your primary care most likely will need an actual referral number listed in order for insurance to process your claim. Most insurances will not accept a referral done afterwards, this is why the information is required beforehand. Please be advised that if you are having a hearing test this will generate two claims; one for the office visit and another for the hearing test. We will bill your insurance for both of the provided services. If your insurance plan also requires a copay for the hearing test, a bill will be sent to you for the balance due. Any surgical procedures performed will generate billing from multiple providers. At any given time approximately 20% of school aged children will have middle ear effusions. By 6 years of age 40% of children will have experienced 3 or more ear infections. Each year approximately 667,000 children under the age of 15 will have tympanostomy (or pressure equalization) tubes placed in their ears. By the age of 3, one in 15 children will have had PE tubes placed. PE tubes are usually indicated if ear infections recur frequently, or if fluid in the middle ear fails to clear. Hearing loss and ineffective oral antibiotics (or troublesome reactions to antibiotics) are also reasons one would consider placement of PE tubes. These tubes have been shown to reduce the presence of effusions by 32% during their first year, and they improve hearing levels between 5-12 dB. Side effects include persistent ear drum perforations after extrusion of the tubes (in about 2% of patients) as well as intermittent drainage from the tubes (in about 20% of patients). Risks of anesthesia are minimal at between 1:10,000 and 1:45,000 patients. There are many approaches to open blocked sinuses. Office based procedures such as balloon sinuplasty offer a minimally invasive approach to alleviating sinus discomfort. It can be accomplished with topical anesthesia in an office setting and with minimal recovery time. Endoscopic approaches are also performed as an outpatient, but they frequently require sedation or a general anesthetic. While a limited amount of work can be performed in the immediate post-operative period, usually a week is required for recovery. Finally external approaches to the sinuses (incisions through the facial skin) may be required for severe infections or in patients who’s symptoms have not responded to previous surgeries. Dr. Hoyt has experience in each of these techniques and he can suggest an individual treatment plan based on each patient’s history and state of disease. This can cause health and quality of life issues for both the affected individual and their partners. It occurs more commonly in men, and it occurs more commonly as people age. It happens when there is some restriction in the free flow of air through the back of the nose and throat. This can occur because of poor muscle tone, excessive bulging of tissue, and obstruction of nasal airways. Sleep Apnea While snoring can be annoying, sleep apnea can cause similar symptoms; yet in addition, it can lead to poor job performance, excessive daytime sleepiness, and danger in operating hazardous equipment and motor vehicles. Sleep apnea has been associated with a higher incidence of cardiovascular disease, heart attack, stroke, and diabetes. A physical examination as well as a sleep study may be necessary to evaluate these concerns. Treatment options include the use of a mask-like device during sleep to increase the air pressure in the upper part of the throat and aid in keeping it open. Other options include surgery to reduce the bulk of the palate and tonsils, as well as procedures to open the nasal airway such as a septoplasty or turbinate reduction. Other procedures can reduce the bulk of the tongue base or realign the position of the tongue base to insure a more open airway.
